Hi Doc, I am a 35 year old Australian male who is currently in Dubai. Last Friday night I got drunk and ended up having vaginal sex with a Chinese prostiute. Luckily I remember what happened and I know that a condom was used and that the whole event lasted about 5-10 minutes.When she removed the condom I felt a pinch at the base of my penis, but when I inspected this the next morning I couldn't detect any broken skin.I also touched the condom briefly when I withdrew(no visible cuts on my fingers).When leaving the prositutes apartment I asked her if she had aids and she said she was clean, but how would you know? My real problem is that I am married with two kids and they will be here next week.I am so paranoid that I have contracted Aids even though I used a condom. Naturally my wife is going to want sex straight away,and I can't suggest to wear a condom or tell her what happened as it would mean the end of the world losing her and my kids. Obviously I can't get tested with in a week of the event,so i am terrified I am going to pass something on to her and even my kids.
1.What was my risk ?
2.Should I get tested and when ?
3.Are do it yourself aids tests available on the internet reliable?If so which ones ?
4. What are the risks having unprotected sex with my wife next week,given that she enjoys anal aswell.
This was totally out of character for me and is killing me. Hope to here from you soon Doc, Thanks.

Relax.  1) You used a condom, therefore no risk.  And she probably doesn't have HIV; most people don't like when asked directly.  The 'pinch' makes no difference.  2) From a medical or risk assessment perspective, you do not need testing at all.  But if my reassurance isn't enough and you need a negative test result to feel better, it's fine with me.  See numerous other threads about timing of testing (4-6 weeks is fine).  Every HIV antibody test on the market, including tests available in Dubai, is sufficiently accurate.  Have whichever test your own health care provider routinely offers.  4) There is no risk to your wife.

Thanks Doc, that was unbelievably quick response !
Forgot to mention I had the flu at the time. I also had the sweats the next few days, but I presume this was because of the worry and guilt. Thanks again.

I happened to be online in the forum when your question came in.

Influenza or similar infections/symptoms do not increase the risk, and sweats by themselves are not a sign of HIV.  When a person suspects his/her own symptoms are due to stress or anxiety, s/he almost always is right.

What makes me say it?  The fact that it's true.  There are no definitive data, of course, but not all commercial sex workers are at high risk of HIV.  In the US and Europe, higher-cost workers probably are no more likely to have HIV than most other sexually active women.  Different story for brothel workers, street walkers, etc.  None of this is hard and fast, of course.
