White bumps on foreskin

Hi,

Around 2 months ago I had sexual contact (not intercourse) with a girl.  I used a condom nearly all the time but there was a 20-30 second period where there was direct contact.  The girl had never had any kind of sexual contact with anyone before and is sure that she has no STD.  Before that my last sexual contact was 9 months earlier, with a condom, and I was tested immediately afterwards.  A few weeks ago i noticed several white bumps on the inside of my foreskin.  There are around 4 that are small round and white and one that is almost donut shaped and pinkish.  I dont think they have changed at all since they appeared, they dont cause any itching, and they dont burn when I urinate.  I have no other symptoms or discomfort.  I went to see a doctor and he said that they looked like they might be syphilis.  He gave me a blood test which came back negative.  I was more worried that it might be herpes or HPV but the bumps aren't sores and he claimed that they dont look like either.  I went back a week later and the doctor took a culture from two of the bumps which came back positive for a yeast like fungus.  He perscribed a cream called clotrimazole to use 3 times daily.  I have now been using it for almost three days and other than dulling somewhat the bumps remain unchanged.  I am still very worried that I might have something more serious.  Is it common for a test to come back positive for a kind of yeast and also to have a different STD?  How likely does it sound that I might have a more serious STD?

The spots that you describe are not consistent with the presentation of any typical STD and certainly are in no way suggestive of herpes (the description is not suggestive and herpes lesions would not still be present for this long).  There is an outside chance that they could be genital warts but your doctor says they are not and they really don't sound like an STD. They could also be normal structures that you had not noticed in the past such as sebaceous cysts or pearly penile papules.  They also could be a slowly responding fungal infection as suggested (a few are not responsive to clotimazole and require a different medication).  It sounds as though you would really like to know what they are and, if need be, what to do about them.  To accomplish this, I would suggest seeing a dermatologist.  While waiting however, please do not worry, these are most unlikely to be something serious and may well be normal.

Dr.,

Thank you very much for the quick response!  As you gathered, I am very worried and am looking to do anything possible to find out what exactly the problem is and then try to resolve it.  The issue is I currently live in a developing country where the medical care is decent but not great.  That is why I am not putting a huge amount of faith in the doctor whom I visited's opinion.  Do you see any problem with me looking for a stronger anti-fungal cream after a week of treatment with clotimazole and switching to that?  If not, should I continue using the same medication for more than 7 days?  It is possible the bumps have diminished slightly though that might just be a normal cycle between morning and night.  I will also search for a dermatologist here to get a more definitive diagnosis.

Switching to a different antifungal would not be a problem. Oral fluconazole, if available might work well.  7 days of therapy should be plenty.  EWH

I have just finished my 7th day of taking oral fluconazole and there has been no change.  I have also noticed that my penis feels a little bit off.  Not itchy, burning, or any other kind of pain though.  It almost feels like there is a bubble inside my urethra.  My foreskin is also very dry...though I suppose that could be because I have been washing and inspecting it constantly.  I have a doctors appointment in a more developed country in a week, is there nothing I can do before then to try and pinpoint what it might be?  Thanks for all the advice you have been able to give me so far and anything from this point forward.

The non-response to fluconazole pretty much rules out a fungal infection.  Sorry, at this point what you need is to be examined.  On the other hand, I do hope that it will be reassuring that your history is not consistent with an STD and as things are sorted out you will likely discover that you have some sort of non-STD dermatological problem.  Please let us know wha tthe doctor says.  EWH
